Concept Map: The second component of the paper will be a concept map describing a plan of care for the patient. This concept map should include a brief overview of the patient, four nursing problems with supportive subjective and objective data, patient goal, nursing interventions, and evaluation of the goal. The nursing problems identified should include:
Two priority nursing problems pertinent to why the patient is admitted to the hospital
One risk for problem: Something that is not an active problem but that the patient is at risk for (falls, impaired skin integrity, etc.)
Knowledge deficit: All patients require education; discuss elements of teaching for the patient
Components of Concept Map:
Background Information: Include patient’s initials, age, allergies, reason for hospitalization, past medical history, and assessment priorities based on the reason for hospitalization and current condition. Example: If the patient is admitted with CHF, your assessment priorities would be cardiac and respiratory systems as well as activity level, I&O, etc.
Nursing Problems: Priority nursing problems for your patient. This should include two priority problems pertinent to why the patient is admitted to the hospital; one risk for problem, and knowledge deficit (see descriptions above).
Patient Goal: Each nursing problem should have a SMART goal. The goal should be attainable, measurable, and associated with the problem. A timeline for evaluation of the goal needs to be established.
Nursing Interventions: These should include medications and other interventions performed to help achieve the goal stated. Although monitoring is important, make sure some of your interventions are actions as well. Include rationale for each intervention (Why are we doing it?)
Evaluation of the Goal: List things you will assess to evaluate if the goal has been met. Include follow-up subjective and objective findings that support whether the goal was met. If the goal was not met, what could you have done differently to help meet the goal?
